Stress fractures & bone stress injuries
Learn why persistent focal pain with activity deserves assessment, and how bone loading and recovery influence rehabilitation.
Stop the painful impact activity and arrange prompt assessment for suspected bone stress pain. Difficulty walking, severe hip or groin pain, or pain that is worsening at rest needs urgent medical advice.
A spectrum of bone stress injury
Bone continually adapts to loading. If repeated demand exceeds its ability to recover, a stress reaction can develop and progress to a fracture. This can occur after a rapid training increase or, when bones are less strong, during relatively ordinary activity.
Recognising the pattern
Pain is often localised and initially linked to exercise. It may progress to pain with daily walking or at rest. Swelling is possible. Tell the clinician about training changes, previous stress injuries, nutrition, menstrual changes where relevant, medicines and bone health. These factors can affect investigation and recovery planning.
Why an early normal X-ray may not settle the question
Early stress injuries may not appear on X-ray. Examination findings may justify further investigation, such as MRI, or precautions while the diagnosis is clarified. Location matters: some sites need more protection or specialist care than others. Do not keep running simply because one early image was normal.
Healing and returning to activity
Management reduces the load on the affected bone and addresses contributing factors. Some people need a boot, crutches or other treatment. Recovery time varies with site and severity. Return to impact should follow clinical guidance and progress gradually; adequate food intake, recovery and appropriate bone-health assessment belong in the plan.
Make your appointment useful
Bring a brief history of the symptoms, relevant reports and your medicines list. Explain which activities matter most to you and what you have already tried. Ask what diagnosis best explains the findings, what would change that diagnosis and how you will measure progress.
Agree on what to do between appointments, how to adjust activity if symptoms change and when to review the plan. Tell your practitioner if treatment causes a new problem or progress stalls.
